Untitled Document The RF Central Office has informed SPS that they will be strictly enforcing the requirement that *middle initials* be included when entering new names for payment (i.e., human subject, participant fee, independent contractor and petty cash) into the RF Business System (please see email below). If the payee does not have a middle initial, then you must indicate this when you fill out the payment request form.

The purpose of enforcing this requirement is to reduce the number of duplicate new entries in the System. If we do not receive the full payee info from the PI or Department, WE WILL BE UNABLE TO PROCESS PAYMENT. Thank you for your cooperation.

Subject: Complete Names and Middle Initials Required

The Supplier File Naming Standards have always required that when setting up a new person that the full name (no nick names) be used - that includes a middle initial. Effective immediately, we are going to start enforcing this requirement to try to reduce the number of new duplicate suppliers being created. If you are being given paperwork without all the information you need to comply with this requirement, you need to inform the requestor that the supplier cannot be set up. Please let me know if you have any questions.

Thank you for your cooperation on this.
