The Veterans Administration Hospital of Western New York (VAWNY) has its own IRB. Approval from both a UB-IRB and the VA-IRB must be obtained under the following circumstances when:

 

·        UB Investigators:

 

o        will be using VA facilities (e.g., CTRC, VA-UB PET Scanner)

o        will be recruiting from VA facilities.

 

·        Investigators with VA appointments:

 

o        will be conducting their research project at a UB-facility (in part or entirely)

o        will have the funds for the research project administered through Sponsored Projects Services (Sponsored Programs Administration - SPA).

 

In the above circumstances it is recommended the shared VA-UB consent form document template be used. However, the VA-UB shared consent form document template can only be used in the circumstances described above. You will need to use separate consent form templates in other circumstances.

 

Other than the shared consent form document template, all other documents that are required by the VA-IRB and the HS-IRB must be submitted. Please check the VA-IRB website for their specific requirements.
