Note: When submitting substantive materials electronically to the SBSIRB, the number of attachments should be minimized by combining all files of the same file extension type (ex. .doc, .pdf) into a single file

By converting to this format the SBSIRB will be able to simplify the paper work by cutting down the number of different application types that need to be used by investigators as well as eliminating the need for redundant signatures. Directions for the use of this file and which pages need to be filled out are on the first worksheet of the workbook.
