Provide investigators with appropriate information to facilitate the ethical conduct of research and review submissions for completeness of materials, adherence to institutional policies, procedures and federal regulations.

The SBSIRB has revised the Excel format form Package required for submission as part of its ongoing efforts to improve the protection of human research subjects and to meet accreditation standards. The new forms use a linear format where all requested information is in one column of a form as well as interactive formatting that highlights items that need to be completed based on information already provided in the form.
The Serious Events and Problems (SEP) form has replaced the Adverse Event Report (AE/SAE) and now includes instructions for determining whether the event/problem is “serious” along with a table of “serious” events/problems and their required reporting timeframes.

Note: When submitting substantive materials electronically to the SBSIRB, the number of attachments should be minimized by combining all files of the same file extension type (ex. .doc, .pdf) into a single file
Substantive Material Preparation
Directions, guides, examples and templates to be used in preparation of protocols, consent documents, recruitment materials, etc. to be submitted as a part of the SBSIRB submittal process.
