Closing a protocol via e-mail
Note: both pdf and doc formats are provided for most items on this site. In order to save any changes you make to downloaded documents, you will need to use the file type compatible with your software editor. Acrobat Reader cannot save changes made to pdf files, you will need Acrobat Writer or other software to do this. Doc files can be edited and saved using Word.
The principal investigator will be sent an e-mail reminder to continue or close
a protocol 1-2 months prior to the expiration date. Included in the e-mail
will be the SBSIRB Form File
. Only the SBSIRB Renewal page needs
to be filled out to close a protocol. To close a study, fill in the principal
investigator name, project title and expiration date. Under Project Status,
indicate "Project Complete." Complete the second page, which consists of protocol
summary information. E-mail the form as an attachment to the SBSIRB (SBSIRB@research.buffalo.edu).
Once the SBSIRB has received the form, an e-mail letter verifying the closure
will be sent to the PI.
Once a protocol has been closed by either the investigator or the SBSIRB, it may not be reopened and a new submittal must be made and approved by the SBSIRB in order to continue the research.
